When advanced gum disease progresses, the supportive gum tissue and bone separate from your teeth, creating deep spaces known as periodontal pockets. These pockets can become hiding places for bacteria, plaque, and tartar.
Once a pocket reaches a certain depth, it becomes difficult to clean with regular brushing, flossing, or even a standard dental cleaning. If left untreated, the bacteria can continue to affect the bone, eventually leading to loose teeth and tooth loss. Periodontal osseous surgery is a foundational treatment designed to clean out these bacterial pockets, smooth the affected bone, and reshape the gum tissue so it can be more easily maintained at home.
Osseous surgery is not the first step in treating gum disease. It is typically reserved for when conservative therapies need extra support. When a patient is diagnosed with periodontal disease, the first-line treatment is usually nonsurgical scaling and root planing, a deep cleaning that removes bacteria beneath the gumline.
After allowing the gums a few weeks to heal, Dr. Haider re-measures the depth of the gum pockets. If the deep cleaning successfully resolves the shallower areas but pockets of 5mm or deeper remain, osseous surgery may be recommended, since pockets at this depth are typically too deep for a toothbrush, floss, or a hygienist’s tool to keep clean on their own.
By reshaping the gum tissue around shallower pockets, osseous surgery helps limit the spaces where harmful bacteria can multiply.
Addressing the infection at this stage can help slow the progression of gum disease and support the bone that keeps your teeth securely in place.

Dr. Haider creates a small access point in the gum tissue to reach the tooth roots and underlying bone, clearing away hidden plaque, calculus, and bacteria.
Advanced gum disease often leaves the surrounding bone rough or uneven, creating spaces where bacteria can hide. Dr. Haider reshapes and smooths the bone back toward its natural contours.
The gum tissue is placed back around the teeth and secured with small, dissolving stitches. Placing the tissue against the newly smoothed bone helps reduce pocket depth.
